The Single Point of Access (SPOA) is a centralized intake process for families with children who have significant behavioral health needs and would benefit from additional supports and services in the community. In collaboration with families, service providers, counselors and schools make referrals to SPOA. The overall goals include fostering independence and improving the quality of life for consumers of behavioral health services.
Families or providers may complete the C-SPOA Application (see below) and return via email to [email protected] or fax at 315-719-8327. Please complete the application as much as possible and ensure the signatures on the releases have been obtained by the appropriate person.
