Clinic will be held in the Medical Arts Building - Ground Floor

Please enter through the main entrance of the Hospital

Instructions:

Please wear a short sleeved shirt
Please bring appropriate identification
Please bring proof of employment and job role
Please bring insurance card/proof of insurance
