[ARCHIVED] Positive Home Test for COVID-19?

If you live in Yates County and have tested positive for COVID 19 using a home test kit, you can report your test to Public Health by completing our HIPPA-compliant, electronic Self Reporting Form and then:
  1. Stay home for 10-days from your first symptom. No symptoms? Stay home for 10-days from the date of your positive test. 
  2. Keep all unvaccinated close contacts home for 10-days from their last unmasked contact with you. Wear a mask if you have unvaccinated people in your home and do your best to stay 6-feet away from them. Vaccinated contacts who don't have symptoms, don't need to quarantine, but should mask while around other people for two weeks.
  3. You will receive a phone call from a NY State Investigator or Contact Tracer to complete your case investigation. This may take up to 48-72 hours.  It will look like an unknown number and will be from a 518 Area Code. Please answer the phone call and consent to a quick interview, ask questions, and follow the instructions they give. 
  4. If you have any questions, you can call our office Monday thru Friday, 8:00am to 4:00pm.
