[ARCHIVED] Positive Home Test for COVID-19?

This self-report form is for Yates County Residents Only.  If you live in Yates County and have tested positive for COVID 19 using a home test kit, you can report your test to Public Health by completing our HIPPA-compliant, electronic Self Reporting Form. You will need to upload a picture of your positive test in the self-reporting form. If you do not live in Yates County, please visit your county local health department website. 
Aft your fill out the form, please: 
  1. Stay home for 5-days from your first symptom. No symptoms? Stay home for 5-days from the date of your positive test. 
  2. Stay in a separate room from other household members and use a separate bathroom, if possible. 
  3. Wear a mask around other people in your home and do your best to stay 6 feet away from them. 
  4. Keep all unvaccinated close contacts home for 5-days from their last unmasked contact with you.
  5. Fully vaccinated contacts (received primary vaccine series and their booster, if eligible)  who don't have symptoms, do not need to quarantine, but should mask while around other people for two weeks.
  6. You may not receive a phone call from NYS or Public Health due to the high volume of cases.  Click Here to print off your isolation order that you can provide to your employer/school if needed.
