Yates County Public Health is looking for volunteers who are caring individuals with good facilitation skills to support and encourage health seeking community members in a Blood Pressure Self-Monitoring Program. Healthy Heart Ambassadors empower people to take control of their blood pressure. Required program training is provided for interested volunteers. 
Healthy Heart Ambassadors provide support and educate program participants to develop the habit of monitoring and tracking their blood pressure through individual consultations during set hours and weekly support messages in order to achieve better blood pressure management. They also encourage program participants to engage in daily physical activity and provide ongoing support as they strive to achieve their program goals.
